Please use the below form to request a water service connection. We will contact you within (48?) business hours after receiving the completed request. All fields must be completed.

Site Utility Plan:

A preliminary site utility plan or drawing must be submitted with this request.

Meter Information:

Please contact 920-459-3814 or metering@sheboyganwater.org for meter sizing. Meter installs require 30-60 day lead time.

An as-built site utility plan must be submitted to have a meter installed.
